Able Palms Home & Health Care Services is a certified home health agency licensed by the State of Florida to provide both skilled and private duty services to adults in their homes throughout Pinellas and Pasco Counties.

Skilled Services

What criteria should be met to qualify for Medicare coverage for home health services? Medicare pays 100% for home health services if the criteria are met:

1. Physician must order home health services as medically necessary and sign Plan of Care
2. Client must be eligible for Medicare Part A services
3. Services are intermittent, reasonable, skilled, and medically necessary
4. Patient must be homebound

Who can benefit from Home Health Care Services?

• Anyone with a new or acute diagnosis
• Anyone who has had a change in treatment or medication
• If you are recently discharged from hospital, nursing home, or rehab facility
• After you have visited an emergency room or physicians office
• If you have had frequent falls

Private Duty Care

Services begin with a free in-home evaluation by a Registered Nurse to discuss the client’s needs and to develop a personalized Plan of Care.

Our employees are carefully chosen and undergo background screening, drug testing, and reference checks for your peace of mind. All care services are supervised by our Director of Clinical Services.

In home personal care services include but are not limited to:

• Bathing and dressing
• Continence care
• Meal preparation
• Transportation, shopping, and errands

Assistance with Insurance Questions

If you have insurance, our office staff will assist you in reviewing your coverage and will assist you with the necessary documentation to present to the insurance company for your reimbursement.

Free In-Home Evaluation

Able Palms Home & Health Care Services’ professional staff will visit any client to perform an evaluation and develop a plan of care directed toward meeting their needs.

On a continuing basis, the Nursing Director will supervise care of the client to ensure appropriate health care delivery, as well as providing nursing guidance and referrals on all aspects of home health care. This service is provided as a courtesy to our clients, for which there is NO CHARGE.

The Range of Services Include:

• Registered Nurses to provide specialized nursing care, including home visits for wound care, injections, supervision, and administration of the treatment plan.
• Licensed Practical Nurses to provide nursing care including dressing changes, injections, diabetic monitoring, instruction, and catheter care, as well as medication set-ups.
• Certified Home Health Aides to assist with personal care, bathing, dressing, ambulation, feeding, and housekeeping.
• Companions to provide assistance to appointments and shopping, meal preparation, and emotional support.
• Physical Therapy
• Occupational Therapy
• Speech Therapy
• Medical Social Worker
